Amazon built an AI recruiting tool to help screen job candidates. The model was trained on historical hiring data. Because the historical hiring data reflected years of decisions made in a male-dominated industry, the AI learned to penalize resumes that included the word “women’s” and downgrade graduates of all-female colleges. Amazon scrapped the tool after discovering it had been actively discriminating in ways no human reviewer had noticed. The AI did not create the bias. It found it in the data and acted on it at scale.
IBM deployed Watson for Oncology at several major hospitals as an AI tool to help recommend cancer treatment plans. Doctors at those hospitals later reported that Watson was making recommendations they considered unsafe or clinically incorrect. An investigation found the tool had been trained on a limited set of hypothetical patient cases rather than real clinical data. The foundation was flawed. The AI was confidently wrong. Memorial Sloan Kettering, which had partnered with IBM on the project, eventually discontinued its relationship.
Air Canada’s AI chatbot told a grieving customer that he could apply for a bereavement fare discount after his flight, as long as he did so within 90 days. The customer followed those instructions, applied after traveling, and was denied. Air Canada argued it was not responsible for what its chatbot said. A Canadian tribunal disagreed and ruled Air Canada liable for the misinformation its own tool provided. The chatbot was not connected to accurate, current policy data. It produced a confident, plausible, and entirely wrong answer.
In each of these cases, the AI tool itself was not the problem. The problem was the foundation underneath it: biased training data, incomplete information, disconnected systems, and insufficient governance.
